Team-Based Evaluation Process

At Ascend Wellness Group, we ascribe to the philosophy that understanding the root of the cause is at least as important and impactful as treating the symptoms.  Our evaluation process is one that we pride ourselves in and is in the mold of being thorough and thoughtful. Drs. Khan and Chen were trained at leading academic institutions, including Stanford, University of Washington, and Emory, and bring this tested evaluation process from their training backgrounds to Ascend. 
 
Prior to the evaluation appointments, you will receive a call from either Dr. Khan or Dr. Chen who will provide a quick, free phone consultation. Based on this, they will select the ideal psychiatric nurse practitioner to evaluate and work directly with you or your loved one. Throughout the process, the nurse practitioner will regularly collaborate with Drs. Khan and Chen, conveying to them observations and pertinent clinical data. At the end of the evaluation process, the psychiatric nurse practitioner will inform the patient (and/or family) of the patient’s diagnoses, treatment options and recommendations.
 
Because Drs. Khan and Chen play a close role in the evaluation process, the patient can be transitioned seamlessly to one of them if it is a better fit. This isn't common and happens approximately one out every 45 cases and the patient does not need to undergo another evaluation process in this instance. 
 
If the patient ends up maintaining their care with us, they typically are under the direct care of their respective psychiatric nurse practitioner, all while being overseen by Drs. Khan and Chen. 


Children & Young Adults:

4 appointments
45 minutes each

1st appointment: patient alone (barring special circumstances)
2nd appointment: parent(s)/Guardian(s) alone
3rd appointment: patient alone
4th appointment: patient & parent(s)/Guardians(s) together

After the third appointment, the clinician performs the last of multiple reviews with Drs. Khan and Chen and creates a unique treatment plan based on the patient’s case in accordance with the parents.

Adults:

2 appointments
45 minutes each

After your first appointment, your clinician will go over your case with our psychiatrists and ensure your treatment plan is tailored to your specific needs.  The second appointment is utilized to go over the assessment and explain to you your diagnosis, treatment plan and what you can expect from us going forward.

Collaborating with Healthcare Providers:

As part of our evaluation process, we find it helpful to speak with any other mental health practitioners you are currently seeing or have seen in the past, with your permission.  Collaborating with those already providing you care helps us factor in any bits of information that didn’t come up during your initial evaluation sessions. Integrating all facets of your healthcare into the treatment plan only elevates the level of care and guidance we are able to provide you.
